Required Forms to be Completed by Parents

Please complete, sign, and return these forms to the ECC via email to Geeta Latchman.

• Policy Statement and Agreement (PDF)
• Research Consent (PDF)
• Pickup Authorization (PDF)
• Application for Child Care Assistance (PDF)
• Income Eligibility Form (PDF)—This form is required by the New York State Department of Health and Mental Hygiene to determine federal funding that the ECC will receive as part of the Child and Adult Care Food Program.
• Topical Ointments Authorization and Release (PDF)
• Authorization and Release for Emergency Medical Treatment (PDF)—This form is required to authorize the ECC to obtain emergency medical care of your child if your child is injured or becomes ill while in the physical custody of the center and the center deems this care to be necessary.

Please have your child’s physician complete, sign, date, and return this form to the ECC via email to Geeta Latchman.

• Child Health Examination Form (PDF)—This form is required by the New York State Department of Health and Mental Hygiene and Department of Education in order to attend the ECC.
