Conservatory of Music Forms
Prospective students may submit an Audition Request Form online below. Those interested in applying for a scholarship should download the appropriate forms below. In addition, the Conservatory Concert Office requires students to complete a variety of forms to present degree and non-degree recitals, to have their recital recorded, and to apply for instrument lockers, each of which can also be obtained from the Concert Office. Below are the various application forms.
Available Forms
- Undergraduate Audition Request Form—All applicants must submit a separate CUNY application to Brooklyn College in addition to applying to the conservatory.
- Graduate Audition Request Form—All applicants must submit a separate Brooklyn College application in addition to applying to the conservatory.
- Undergraduate Scholarship Application (PDF)—Partial tuition scholarships for deserving applicants on the basis of ability or need.
- Undergraduate Scholarship Reference Form (PDF)—Include when submitting your Scholarship Application. (Two are required.)
- Graduate Scholarship Application (PDF)—Partial tuition scholarships for deserving applicants on the basis of ability or need.
- Graduate Scholarship Reference Form (PDF)—Include when submitting your Scholarship Application. (Two are required.)
- Recital Application Form (PDF)—For undergraduate and graduate degree recitals as well as nondegree recitals. This form should be brought to Pre-Recital Jury.
- Concert Recording Request Form (PDF)—To be returned to George Brunner, director of music technology.
- Locker Application Form (PDF)—Lockers are assigned on a first-come/first-served basis, although preference is given to instrumental music majors for storage of their instruments.
